High school sports are nearly back. Make sure you have all the fees, forms and physicals you need in order to play ball!

PMG FILE PHOTO - High school sports are nearly back, with fall practices starting across the state next Monday, Aug. 15. Make sure to get all of your fees, forms and physicals squared away before the season begins!Believe it or not, it's almost time for high school sports to start up again.

The first official practices of the 2022 fall season begin Monday, Aug. 15, for football, volleyball, soccer and cross country. In order to get on the field, each student-athlete must be sure to get their fees, forms and physicals in order before taking the field.

For prospective athletes at Madras High School, the school's athletics website has links to all the necessary forms at www.madrasathletics.com, under the tab labeled "Forms" in the menu bar.

Those forms include an athletic registration form, single-trip permission slips and physical exam forms in both English and Spanish language.

There are also links to the OSAA's intent-to-transfer certificate, a school representation eligibility certificate (for students wishing to play sports in at a different school than the one they attend) and a checklist for home-schooled students to ensure eligibility.

The Madras Athletics website also features an electronic copy of the MHS athletic handbook, links to the Jefferson County School District 509-J concussion protocol for contact sports and other information under the "More" tab on the menu bar.
